CASO EDWIN DANILO/ EDWIN DANILO CASE

CASO “EDWIN DANILO”, de 14 años, vive en Bogotá, Colombia.

Ingresa: Hospital Cardiovascular del Niño en Soacha, en la UCI.

Diagnóstico inicial: Hipertensión Pulmonar Severa.

Tratamiento:

• 31 de marzo se le instala un catéter (para pasar medicamentos), pero el procedimiento falla perforando un pulmón provocando un neumotórax.

• 02 de abril se realiza cateterismo con diagnóstico reservado.

• 07 de abril junta médica se da inicio a tratamiento farmacológico:

  1. Treprostinil solución inyectable 1 mg/ml + kit de mantenimiento (subcutánea)
  2. Bosentan tabletas 62,5 mg (31,5 mg c/24 horas)
  3. Espironolactosa 25 mg (25 mg c/24 horas)
  4. Furosemida 40 mg (20 mg c/24 horas)
  5. Omeprazol cápsula 20 mg (20 mg c/24 horas)
  6. Sildenafil tableta 59 mg (20 mg c/8 horas)
  • 09 de abril se inician los trámites para conseguir los remedios de alto costo a través de la aseguradora de salud correspondiente (que, al no ser el asegurado directo, sólo cubre un porcentaje).
  • Del 09 al 14 aún no existe respuesta positiva; de no haber una respuesta se procederá a poner un recurso de protección en favor del menor.


Esta enfermedad requiere del tratamiento farmacológico de por vida del menor, ya que su tratamiento es PALIATIVO.

Case of “Edwin Danilo,” 14 years old, lives in Bogotá, Colombia.

Admitted to: Cardiovascular Hospital for Children in Soacha, in the ICU.

Initial diagnosis: Severe Pulmonary Hypertension.

Treatment:

• On March 31, a catheter was inserted (to administer medication), but the procedure failed, perforating a lung and causing a pneumothorax.

• On April 2, catheterization was performed with a guarded prognosis.

• On April 7th, the medical board initiated pharmacological treatment:

  1. Treprostinil injectable solution 1 mg/ml + maintenance kit (subcutaneous)
  2. Bosentan tablets 62.5 mg (31.5 mg every 24 hours)
  3. Spironolactone 25 mg (25 mg every 24 hours)
  4. Furosemide 40 mg (20 mg every 24 hours)
  5. Omeprazole capsules 20 mg (20 mg every 24 hours)
  6. Sildenafil tablets 59 mg (20 mg every 8 hours)
  • On April 9th, the process began to obtain the high-cost medications through the corresponding health insurance provider (which, since the patient is not the direct insured, only covers a percentage).
  • From the 9th to the 14th, there was still no positive response; if there is no response, a protective order will be filed on behalf of the minor.

This disease requires lifelong pharmacological treatment for the child, as its treatment is PALLIATIVE.
